如何有效降低急性ST段抬高型心肌梗死(STEMI)的各种并发症和死亡率,是医学界的研究热点。依据近期的大型临床试验,国内外不断更新急性心肌梗死的治疗指南以便更好的指导临床工作。2012年8月欧洲心脏病学会(ESC)公布的《急性ST段抬高性心梗处理指南》以及2012年12月美国心脏病学基金会(ACCF)和美国心脏协会(AHA)联合发表的《2013年美国ACCF/AHA STEMI治疗指南》中,有很多新的亮点,例如急性心肌梗死的处理流程、再灌注治疗以及抗栓、抗凝治疗等等。本文通过简析上述最新治疗指南来概述急性ST段抬高型心肌梗死的治疗新进展。
Abstract：How to effectively reduce acute ST-elevation myocardial infarction( STEMI) complications and mortality is a hotspot of the medical profession. Based on the recent published large-scale clinical trials,Guidelines for the treatment of acute myocardial infarction has constantly updated in order to give better instruction of clinical work. European Society of Cardiology( ESC) announced《The Guidelines for the management of acute myocardial infarction in patients presenting with ST-segment elevation》in August 2012,and in December 2012,American College of Cardiology Foundation( ACCF) and the American Heart Association( AHA) jointly published《2013 ACCF / AHA Guideline for the Management of ST-Elevation Myocardial Infarction》. There were many highlights in these Guidelines,such as treatment process of acute myocardial infarction,reperfusion,antiplatelet therapy and anticoagulant therapy. This paper is a overview for the treatment of STEMI according to above guidelines.
