目的 探讨间歇性θ脉冲刺激(iTBS)联合持续被动运动训练对前交叉韧带(ACL)损伤术后患者康复效果的影响。方法 选取2021年1月至2024年7月九江一七一医院收治的80例接受关节镜下韧带重建术治疗的ACL损伤患者。男性60例,女性20例,年龄(31.1±6.5)岁,年龄范围为19~51岁。根据术后不同的康复方式分为对照组(n=39)和观察组(n=41)。对照组接受持续被动运动训练,观察组在对照组基础上加用iTBS干预,两组患者均连续干预6周以判定干预效果。比较两组患者术后早期疼痛程度、干预前后膝关节本体感觉及膝关节功能。结果 两组患者均无重度疼痛,对照组中度疼痛的患者比例高于观察组,差异有统计学意义(P<0.05)。干预前,两组患者的患膝30°、60°被动成角误差比较,差异无统计学意义(P>0.05);干预6周后,两组患膝30°、60°被动成角误差均减小,观察组患膝30°、60°被动成角误差小于对照组,差异有统计学意义(P<0.05)。干预前,两组患者的特种外科医院膝关节评分(HSS)比较,差异无统计学意义(P>0.05);干预6周后,两组HSS评分均较干预前升高,且观察组HSS评分高于对照组,差异有统计学意义(P<0.05)。结论 iTBS联合持续被动运动训练可减轻ACL损伤术后患者术后早期疼痛,提升膝关节本体感觉,并促进患者膝关节功能恢复。
Abstract:Objective To investigate the effect of intermittent θ pulse stimulation(iTBS) combined with continuous passive exercise training on the rehabilitation outcomes of patients with anterior cruciate ligament(ACL)injuries after surgery. Method A total of 80 patients with ACL injuries who underwent arthroscopic ligament reconstruction surgery at the jiujiang 171 hospital,from January 2021 to July 2024 were selected. There were 60 male patients and 20 female patients,with an average age of(31.1±6.5)years old,ranging from 19 to 51 years old. According to different rehabilitation methods after surgery,they were divided into the control group(n=39)and the observation group(n=41). The control group received continuous passive movement training,while the observation group received additional iTBS intervention on the basis of the control group. Both groups were continuously intervened for 6 weeks to determine the intervention effect. The early postoperative pain degree,knee joint proprioception before and after intervention,and knee joint function of the two groups were compared. Results Both groups of patients did not experience severe pain. The proportion of patients with moderate pain in the control group was higher than that in the observation group,and the difference was statistically significant(P<0.05). Before the intervention,there was no statistically significant difference in the 30° and 60° passive angulation errors of the affected knees between the two groups(P>0.05);after 6 weeks of intervention,the passive angulation errors of the affected knees in both groups decreased,and the passive angulation error of the affected knee in the observation group was smaller than that in the control group,with a statistically significant difference(P<0.05). Before the intervention,there was no statistically significant difference in the hospital for special surgery knee score(HSS)scores between the two groups(P>0.05);after 6 weeks of intervention,the HSS scores of both groups increased compared to before the intervention,and the HSS score of the observation group was higher than that of the control group, with a statistically significant difference(P<0.05). Conclusion The combination of iTBS and continuous passive exercise training can alleviate early postoperative pain in patients with ACL injuries,improve knee proprioception,and promote knee joint function recovery.
